Health systems in Europe and around the world are evolving to adapt to the societal and technological changes and the changing health needs as more people live longer with chronic or degenerative conditions. This requires changes in the health systems’ design and new ways of delivering healthcare. Next to reduced mortality, improving general health outcomes and quality of life is gaining importance as the main objective of healthcare.
All stakeholders acknowledge that a shift of emphasis is required from a siloed budgetfocused approach towards a more holistic health and social care system approach. Future healthcare will be more personalized in all stages of healthcare where an evidence-based integrated approach will be applied with an increased role of patients in managing their own health and care.
The conference aims at raising awareness and knowledge among the healthcare community on the value of patient-centered healthcare.
At the conference, good practices and practical examples will be presented by speakers from Estonia and other countries. These ideas will be focused on developing and implementing patient-centred healthcare approaches.
